IMPORTANT TAX DOCUMENT INFORMATION
Form 1095-C, Employer-Provided Health Insurance Offer and Coverage, is available upon request.
Employees and other individuals who are eligible to receive Form 1095-C may request a copy by contacting:

Requests for Form 1095-C may be made by phone, email, or in writing.
Upon request, Form 1095-C will be provided within 30 days of the request or by March 2, whichever is later.
This notice will remain available through October 15 of the applicable tax year, in accordance with IRS regulations.
